Fifth graders will be studying different instruments in units this year.  In addition to playing the recorder, each
student will have opportunities to study keyboard, stick dulcimer, dulcimer, and ukulele.  We have scheduled these
units so each student will have the opportunity to borrow these instruments to take home for practice.  Instruments
may not be transported on a school bus for safety reasons and may not be transported on bicycles.  They may be
borrowed if a parent picks up the student as a car rider or the student walks home.

The maximum checkout time will be one week from the day the class meets.  This will insure the instruments will be
available for use in class the following week.  If a student fails to return an instrument by the due date, he/she can
loose the privilege of borrowing instruments.  Students will also have textbooks issued to them for these units.

These instruments are relatively inexpensive when compared to band instruments.  They range in cost from $5 for a
stick dulcimer, $25 for keyboards, $35 for ukuleles, and $60 for Appalachian dulcimers.

In order to make this equipment available for student use, parents and students must take the responsibility for their
care, just like materials from the library.  Parents will be responsible for paying for a replacement if the instruments
or text is lost or damaged.  These instruments/texts are not toys.  We strongly discourage allowing younger sibling’s
access to this equipment.  While they are in your possession, they should be stored in a safe location, away from
moisture and extreme temperatures.  They cannot be left in a parked car because of temperature fluctuations.

We must have the attached agreement signed by the parent before instruments can be loaned to students.  If you
agree to accept this responsibility, please complete the attached form accordingly and return it to Mrs. Bouton in the
music room.

Thank you for your interest in your child’s musical progress.

I agree to accept responsibility for any instrument and/or text that is borrowed from Graceville Elementary.  If the
instrument or text is damaged or lost, I agree to pay for repair or a replacement.  I also understand that any
instrument borrowed must be returned to the music room by the next week’s music class.
